To apply for, or replace, your Vocational Education & Training (VET) certificate or Statement of Attainment issued by ADAS in accordance with the requirements of the Australian Qualifications Framework (AQF), please complete the AQF replacement form below.AQF Certificates and Statements of Attainment cost $36 + GST (Australian destinations only) + postage for each document ordered.
NOTE: The form will take approximately 5-10 minutes to complete. The form can NOT be saved so please allow sufficient time and ensure that you have all your files to complete the process.
